"What Jane Austen Ate and Charles Dickens Knew" has a pretty good description of the hierarchy of servants. (Although some readers will tell you to take this book's assertions with a grain of skepticism.) As far as names, most 19th century fiction that I've read does indeed have masters referring to servants by their last names. Also, housekeepers were always called "Mrs. Whatever" even if they weren't married.
