] I can't quote a source on this, but I thought that a gentleman was never supposed to take a woman's hand to kiss it, he can only do it when she offers it--it's her move.
It would be rude not to offer her hand. A slight, sort of? It was the way of greeting or?
So could he choose whether he wanted to kiss her hand /the air above it/ or just shake hands ?? Come to think of it, I suppose he could bend his head more or less. It is an act of respect.
