] I actually thought that the monarch could create titles right up to and including Dukedoms (which I _think_ is the highest). The Duke of Marlborough, for example, got that title because of service in the wars of the Spanish Succession. The title would _then_ be inherited, but it had to be created at _some_ point.

The most famous "new" title to come about in the Regency was that of the Duke of Wellington.

He was awarded the title for his victories on the Pennisula Wars, but he worked his way up from Sir, to Viscount to Duke.

In the end, he outranked his older brother who was an Earl.
