] I think that Lady Middleton worries much more about how elegant she appears when the family is in town.

Lady Middleton is also thinking of associating with Mrs. Willoughby because she is a woman of 'elegance and fortune'--regardless of how Marianne has been hurt. And Lady M. sees enough fashion in John Dashwood when they are introduced to think his 'acquaintance worth having'. It's all to do with appearance and nothing to do with the actual feelings, personalities or inclination of involved parties.
