I never thought of doubting that Sophia had dictated the letter, but of course now that you mention it, why should we trust Willoughby on this point?

I think the truth may be some combination of dictation and Willoughby's own words. For example, she may have dictated almost all of it, but he wrote the last sentence. Or he may have written it, subject to her revision and approval. She may have revised his draft a good deal, or just the prospect of her review made him write such stiff, ugly words.

I suspect that the "your most obedient humble servant" part was standard-issue politeness, meaning nothing. But I'm no expert.
