I take it you mean Eliza II and pregnancy. The resemblance between Eliza I and Marianne is openly mentioned ("... Hers, for me, was, I believe, fervent as the attachment of your sister to Mr. Willoughby, and it was, though from a different cause, no less unfortunate. ..." --ch31), so isn't so much of a subject for wondering.

It's only a small lapse, if Colonel Brandon has a moment of visceral alarm and forgets to say hello. For me, that's far more plausible than his going through a longer thought process, speculating about the cause of Marianne's altered looks, guessing at pregnancy, and then not taking a little more time to censor himself.
