The distinction would be Jane Austen's Written by Leah Noel (9/15/2009 6:29 a.m.) in consequence of the missive, What has Mrs. Smith's last name to do, penned by Graciela

I am not of the opinion Willoughby did not respect his cousin because her last name was Smith.

I was however conjecturing that JA named W's relation Mrs Smith to show us, the reader, how he didn't pay her the attention that she deserved and only treated her with his personal brand of common courtesy.
