more like I would have to someone in the modern era saying impulsively..."oh, wow - you look terrible! Is there anything I can do? Can I get you a glass of water? An aspirin?"

No, it isn't really polite, but it is the sort of thing that gets said when one is caught off guard by someone looking very upset, especially someone we care about. And the sort of vaguely foolish thing that tends to fall out of the mouth when we really want to help but have a helpless feeling that either we don't know what to do or there is nothing we can do.
