I think we can give him the benefit of the doubt, and say that he used the name "Miss Elliot". If he'd just jerked a thumb in Anne's direction and asked "Does madam never dance?" , then I imagine that even a Miss Musgrove would be delicate enough to be taken aback at his manner.
