that Mrs Musgrove had the same right as a pretty little slender thing to grieve but that onlookers would not be as sympathic because she was larrge and would tend to see her as a joke.

I read it more as a comment on the onlooker and a reminder we all, whether pretty or plain ugly, have a right to grieve and that the passage showed Wentworth as the kind hearthearted person he was, showing concern rather than sneering at the large fat woman crying over a replusive son.
