I had to laugh about the Pilares. I am one of many women in my family named Pilar, though it's a middle name for me (I am only half-Spanish). Like most families, we keep each other straight with different nicknames: Pilitina, Pilarona, etc. But I have the same problem in the states: I share the same first name with numerous close relatives on my father's side. Interestingly, my American family keeps everyone straight by using the middle name in family situations, and reserving the first name for school and work.
