I think it is simply a plot flaw from Austen, who has ot make Fanny aware of events in london and has to make it claer to us and to Fanny that Mary is very much at fault...so she has no other way of letting us know this... but I question whether Mary, wrong minded as she often is, would be quite so foolish as to make a joke about bribing physicians to Fanny..
