I do think it quite plausible that his infirmity comes from his age and habits rather than actual current illness. He is not deliberately manipulative, but he certainly uses the privileges of age/health/status to get people to do what he wants, or at least to pretend to do what he wants.

If he was a truly awful person, people wouldn't humor him so much. His selfishness seem to be blended with benovelence and gentleness. He is a really complex character to figure out.
