VALETUDINARIAN, (Val-le-tu-de-na'-re- an) n. >. One who is weakly, sickly, or infirm of health.

It doesn't say that there were no real illness.

And I don't see why a person "who thinks constantly or anxiously about his health" could not be really sick.
